Query for Name

I am working from past 1 Year in Canada.
In my Indian Passport my surname is Blank and my whole Name "XXXXX XXXX XXXX" is mentioned in Given Names.

In my first extension of Work permit I filled the WP Extension form similarly as mentioned in Passport,but When I received my Work Permit, Surname was "XXXXX XXXX XXXX" and Given Names was Blank,where I requested CIC to change accordingly as Per Passport.
CIC sent me new copy of WP where they added a line in Footer of WP page as "XXXXX XXXX XXXX" should be read as in Passport without changing in the name fields mentioned in WP.

Now I have applied through on-line for further extension through Representative.The Representative filled the form Behalf of me with "XXXXX XXXX XXXX" as Surname and Given name NULL.
I dint changed intentionally anything as with the same format my earlier WP was Approved.

Question

1) Will it create any problem to get extension as I followed the same name format which CIC followed for my earlier Work Permit but still It doesn't matches with my Passport.

Could you Please resolve this Query of mine.

Is it because Surname is the mandatory field to be filled.I hold an US tourist visa where Surname has my whole name and given names as FNU(First name Unknown)

Please confirm if its true,coz above comment scenario points to the Logic mentioned.

It's not possible to predict the outcome of the case. Using the same presentation of the name as was earlier approved would appear to make the most sense however.
