I am working on L1 visa in USA and have job offer from a canadian firm.
I am married and having one kid and wife.
Me and my wife both are post graduate.
I have LMO letter too and I want to apply for workpermit
My Questions are as follows:

How many forms do I need to fill up for this work permit?
Please specify forms name/number individually for me , wife and kid.

What should be the fees which I need to pay for this work permit.?

What is the time frame in which I get the decision of the embassy?

What are the additional documents which I need to put in along with the application?

I would appreciate if I get the answers ASAP.
