I am currently in Canada on a NAFTA Professional Work VISA. I am looking to extend it this month (May) as it is up July 2nd 2010. I am wondering how the process works for an extension and if a new medical will be required. I am planning to go to a port of entry (Pacific Crossing), where my VISA was initial given, will this work or do I need to mail in my request. My second question is, I work with children and had a medical done before receiving my initial VISA, but as a healthy young women will I be required to again have a medical by a designated doctor?

Extensions are conducted solely via inland processing at the Case Processing Centre in Vegreville, AB. In such an extension application, I would not expect a request for a new medical examination. If you apply for a new work permit via a port of entry, proper procedure would suggest the need for a new medical examination.

Thank you for the information. If I were to send in application for an extension which form would I use? Is it the IMM1249E, Application to Change Conditions, Extend my Stay or Remain in Canada form? Just want to make sure this form applies to me with a NAFTA Professional VISA. Thanks.

The 1249 is the correct form in the situation described.
